<br/><br/> The aim of this study was to assess knowledge, attitude and practice of nurses towards nursing documentation of nursing care plan. This is a descriptive cross section study which was carried ou toexmine knowledge, attitude and practice of nurses towards nursing documentation. Both quatitative and qualitative methods were used. The study was carried out at three different hospitals namely; Choma general hospital, Kitwe central hospital, and  Senanga district hospital.
